A simple enrollment packet can be acquired from a Store-A-Tooth participating dentist or downloaded, printed, and taken to a Store-A-Tooth participating dentist. If your dentist or oral surgeon is not a participating Store-A-Tooth dentist, please call 1-877-867-5753 or contact us for information on acquiring a kit to bring to your dentist.

The enrollment package includes the following sections.

Informed Consent. The Informed Consent describes details regarding the collection, receipt, processing, storage, maintenance, required testing and use of dental pulp. It documents your understanding and consent to the services outlined as well as the collection of your dental pulp by your dentist.

Client Service Agreement. The Client Service Agreement outlines the services provided by our laboratory, NECC.

Payment Authorization. The Payment Authorization form provides Store-A-Tooth with payment processing information as well as your authorization.

Your dentist’s administrator will ensure your enrollment package is completed and provide you with a copy. After your or your child’s teeth are extracted, the administrator will send the tooth and enrollment package to Store-A-Tooth on your behalf.
