The enrollment packet includes:
Informed Consent.
The Informed Consent describes details regarding the collection, receipt, processing, storage, maintenance, required testing and use of dental pulp. It documents your understanding and consent to the services outlined as well as the collection of your dental pulp by your dentist.
Client Service Agreement.
The Client Service Agreement outlines the services provided by our laboratory.
Payment Authorization.
The Payment Authorization form provides Provia Labs with payment processing information as well as your authorization."
Step 2: Bring this kit to your dentist the day the teeth are coming out, and your dentist will help you collect and send your teeth overnight to the laboratory.

Tooth Collection & Shipment:
If your child has a loose baby tooth or an expected wisdom tooth extraction, and your dentist or oral surgeon is a Store-A-Tooth participating dentist, contact them to notify them that you will bring a completed Store-A-Tooth enrollment form or will complete one at the time of visit.
If your dentist or oral surgeon is not currently participating, it is a simple process for Store-A-Tooth to contact your dentist and provide them with the information that is require. Please contact Store-A-Tooth at 1-877-867-5753 or Contact Us and a Store-A-Tooth customer service representative will take it from there.
Store-A-Tooth services are only available for baby and wisdom teeth that are extracted by dentist professionals using carefully designed Store-A-Tooth handling techniques.
Store-A-Tooth shipping protocols and materials provided to participating dentists, as well as the software used to manage your sample, ensure accurate chain of custody from extraction-to-storage.
Your sample will be placed in the Save-A-Tooth® transport device together with a copy of your enrollment package and shipped by your dentist’s administrator. Both the sample and the enrollment package will be bar-coded to ensure accurate tracking of your sample from extraction to storage.
Step 3: Our lab will receive the sample, collect the cells inside, and cryopreserve the tissue. You'll get a report detailing the status of the cells. The stem cells will be cryopreserved until you need them.

Receive Confirmation
Upon receipt, the Store-A-Tooth technicians will inspect your teeth for cell viability.
Teeth that are not viable will be discarded, and you will not be charged for processing and storage.
The Store-A-Tooth lab will send you an inspection report indicating the results as pass or fail.
If the sample passes the viability inspection, it will be frozen at a computer-controlled rate and stored in liquid nitrogen tanks. The process ensures the safety of your cells because progressively lowering the temperature keeps the cells from going into shock before reaching the storage temperature of -321° Fahrenheit (-196°C).
The frozen dental stem cells are stored in tanks located in Store-A-Tooth’s constantly monitored, secure laboratory facility.
